Job summary

Qualifications

Responsibilities

• Coordinate clinical and psycho-social activities with Interdisciplinary Team and Physicians.
• Assist with care coordination and efficient care facilitation.
• Refer high-risk patients for additional support; advocate for patients.
• Participate in interdisciplinary rounds or conferences; collaborate on plans of care.
