Job summary

Banner Health is seeking a Case Manager in Care Coordination to provide comprehensive care coordination for patients across units. The role requires RN with case management knowledge or a Master’s in Social Work with LMSW/LCSW, plus BLS certification for acute settings.

You will support discharge planning, collaborate with physicians, payers, and the health care team, and help empower patients and families in the discharge process. This full-time day position offers diverse hospital exposure.

Qualifications

• RN: knowledge of case management or utilization review typically through Bachelor's degree in case management or health care.
• Social Worker: MSW with LMSW/LCSW license or ability to become licensed within 6 months.
• Basic Life Support (BLS) certification required for assignments in acute care settings.

Responsibilities

• Provide comprehensive care coordination for patients across the hospital.
• Lead discharge planning and communicate plans to patients, families, and the medical team.
• Collaborate with physicians, payers, and staff to meet regulatory requirements.
• Supervise other staff as needed and participate in process improvement initiatives.
