Job summary

Qualifications

RN: must possess current RN license in state worked.
MSW: Master's in Social Work with LMSW or LCSW or eligible to license within 6 months.
Basic Life Support (BLS) certification required.
2 years of clinical experience preferred.

Responsibilities

Coordinate care across the health care continuum to optimize outcomes.
Develop and manage interdisciplinary discharge plan and communicate it across care teams.
Collaborate with physicians, payers, and health care staff to meet regulatory requirements.
Educate team on case management concepts and care coordination processes.
Identify barriers to safe discharge and arrange post-acute services.
