The Jane Pauley Community Health Center, Inc seeks a Masters Level Behavioral Health Therapist in Anderson, Indiana. You will provide psychosocial assessments, counseling, and crisis intervention while collaborating with a multidisciplinary team to promote integrated, patient-centered care.

Joining our FQHC, you will support medically underserved communities, maintain detailed EHR documentation, and engage with community partners to ensure seamless transitions across care levels.

Qualifications

• Master's degree in Social Work, Counseling, or Marriage & Family Therapy.
• Experience in healthcare or mental health services preferred.
• Knowledge of psychosocial assessments and crisis intervention.

Responsibilities

• Conduct psychosocial assessments and develop treatment plans.
• Provide short-term counseling and crisis intervention.
• Collaborate with care team to ensure integrated care.
• Coordinate services across levels of care.
• Maintain clinical documentation in the EHR.
• Engage with community resources and protective services.
• Participate in team meetings and quality improvement.
