Job summary

Qualifications

• Master’s degree in Social Work, Counseling, or Marriage & Family Therapy.
• Active Indiana licensure as LCSW, LMFT, or LMHC.
• CPI (Non-Violent Crisis Intervention) certification preferred.
• Experience in healthcare, mental health, or community-based services.
• Knowledge of disease processes, human behavior, and development.
• Ability to provide effective therapeutic counseling and crisis intervention.
• Strong interpersonal and communication skills; team-based collaboration.

Responsibilities

• Conduct comprehensive psychosocial assessments and develop individualized treatment plans.
• Provide short-term therapeutic counseling and crisis intervention to patients and families.
• Collaborate with physicians, nurse practitioners, and the broader care team to ensure integrated care.
• Coordinate services and support smooth transitions across levels of care.
• Maintain accurate and timely clinical documentation in the EHR.
• Apply knowledge of protective service mandates for adults and children.
• Utilize de-escalation and crisis intervention techniques when needed.
• Build and maintain relationships with community resources and protective services.
• Participate in team-based care meetings and continuous improvement initiatives.
