The Jane Pauley Community Health Center, Inc. is seeking a Behavioral Health Therapist (LCSW/LSW, LMHC/LMHCA) to join our team serving Indiana's medically underserved communities. You will provide psychosocial assessments, counseling, and crisis intervention as part of an integrated care team.

Working here means collaborating with Outreach Specialists, Care Managers, and Patient Care Coordinators to promote access to compassionate, whole-person care in a Federally Qualified Health Center setting.

Qualifikationen

• Master’s degree required in social work, counseling, or related field.
• Active Indiana licensure as LCSW/LSW, LMFT/LMFTA or LMHC/LMHCA required.
• CPI certification preferred.
• Experience in healthcare, mental health, or community-based services.
• Strong interpersonal and communication skills; ability to collaborate in a team-based setting.

Aufgaben

• Conduct psychosocial assessments and develop treatment plans.
• Provide short-term counseling and crisis intervention.
• Collaborate with physicians and the care team for integrated care.
• Coordinate services across levels of care.
• Maintain accurate clinical documentation in the EHR.
• Apply knowledge of protective service mandates for adults and children.
• Utilize de-escalation and crisis intervention techniques.
• Build relationships with community resources and protective services.
• Participate in team care meetings and quality improvement.
