Job summary

Jane Pauley Community Health Center seeks a Behavioral Health Therapist (LCSW/LSW, LMHC/LMHCA) to provide psychosocial assessments, counseling, and crisis intervention for patients in Indiana. You will work within an interdisciplinary team to support integrated care in a medically underserved community.

Qualifications

• Master’s degree in a relevant field is required.
• Active Indiana licensure as LCSW/LSW, LMFT/LMFTA or LMHC/LMHCA is required.
• CPI certification is preferred.
• Experience in healthcare, mental health, or community-based services is needed.
• Strong communication and collaboration skills within a team-based setting.

Responsibilities

• Conduct psychosocial assessments and develop individualized treatment plans.
• Provide short-term counseling and crisis intervention to patients and families.
• Collaborate with physicians, nurse practitioners, and the care team for integrated care.
• Coordinate services and support transitions across levels of care.
• Maintain accurate clinical documentation in the EHR.
• Apply knowledge of protective service mandates for adults and children.
• Utilize de-escalation and crisis intervention techniques as needed.
• Build relationships with community resources and protective services.
• Participate in team-based care meetings and continuous improvement initiatives.
