Vanderbilt Health's VIP Adult - Clarksville unit seeks an Advanced Practice Clinician (NP/PA) to manage a distinct patient population with both outpatient and inpatient duties. You will work with the physician team, ensure adherence to clinical pathways, and demonstrate professional development and competency validation.

Responsibilities include conducting comprehensive assessments, ordering tests, coordinating care, and documenting progress.

Qualificações

• Master's degree required in nursing, physician assistant, or related field.
• Licensed Nurse Practitioner in Tennessee or Physician's Assistant Certification.
• Relevant work experience as indicated by 1 year experience.

Responsabilidades

• Assessment of Health Status.
• Performs and documents a complete history and physical examination for acute and complex chronically ill patients.
• Orders and collects data using appropriate assessment techniques and diagnostic procedures.
• Serves as first responder for patient status changes and provides procedural and resuscitative interventions as needed.
• Coordinates daily clinic schedule with attending physician team and communicates patient status.
• Formulates Plan of Care addressing complex health care needs.
• Prescribes diagnostic strategies and therapeutic interventions, including pharmacologic and non-pharmacologic options.
• Maintains ongoing communication with interdisciplinary health care team and with patient and family.
• Documents key components of patient progress and discharge summaries in the medical record.
