Job summary

Vanderbilt Health seeks an Advanced Practice Clinician (NP/PA) to support both outpatient and inpatient care within the VIP Adult – Clarksville area. The APC will work with the physician team to manage a distinct patient population, ensure adherence to clinical pathways, and maintain high standards of documentation and safety.

Qualifications

• APC may perform outpatient and inpatient care and facilitate discharges/outpatient follow up.
• Strong foundation in evidenced based practice/education and adherence to clinical pathways/protocols for safe quality care.
• Advanced medical and nursing knowledge, clinical/technical competence, judgement, communication, and professional development.

Responsibilities

• Assess health status and perform H&P for acute and complex chronically ill patients.
• Order and collect data with diagnostic procedures as indicated.
• Serve as first responder for status changes and perform procedures as needed.
• Coordinate daily clinic schedule with attending physician team and present patient reports.
• Manage diagnostic tests and formulate plans of care.
• Prescribe diagnostic strategies and therapies to achieve outcomes.
• Document progress and discharge summaries in the medical record.
• Participate in Grand Rounds, APN council meetings, and related professional activities.
