Job summary

Rosecrance Health Network in Champaign, IL seeks a CCBHC Case Manager (Assertive Community Treatment) to provide comprehensive mental health rehabilitation services to adults and families. The role emphasizes evidence-based practices, crisis intervention, and coordinating care to reduce hospitalizations and support community functioning.

Responsibilities include direct counseling, case management, documentation, and collaboration with a multidisciplinary team.

Qualifications

• Requires Bachelor’s degree in psychology, counseling, social work or related field.
• Must qualify as an MHP per Illinois DMH Rule 132.
• Excellent written and oral communication skills.
• Ability to quickly establish rapport with clients and families.
• Proficient with computers and electronic health records.
• Ability to complete documentation within prescribed time frames.
• Skilled in individual, family and group counseling.
• Ability to present and coordinate lectures for clients, parents, staff or community resources.
• Self-starter who can work independently but also collaborate with a team.
• Valid driver’s license and ability to meet agency insurance requirements.

Responsibilities

• Perform primary case management for designated clients.
• Provide crisis intervention and monitoring of medications as needed.
• Conduct individual, group and family counseling sessions.
• Ensure client records are maintained per regulations.
• Coordinate service delivery with community resources.
• Participate in on-call rotation and team meetings.
• Deliver workshops and trainings to clients and staff.
• Adhere to Rosecrance Code of Conduct and legal requirements.
