Job summary

WellMed, part of the Optum family, seeks a Social Worker to assess, plan and coordinate care for patients with complex needs. You will partner with physicians and teams to advocate for patients while ensuring CMS and Medicare guidelines compliance.

Required include a Bachelor of Social Work and 3+ years in healthcare social work; Master’s preferred. Strong MS Office skills, licensure maintenance and reliable transportation are essential.

Qualifications

• Bachelor of Social Work with minimum 3 years’ relevant healthcare experience.
• Current BLS certification or must obtain within 30 days of hire.
• 3+ years social work in a healthcare environment required.
• Proficiency in Word, Excel, Notes, Outlook and related programs.
• Valid driver’s license and reliable transportation for travel.

Responsibilities

• Perform comprehensive evaluations and document findings per CMS standards.
• Conduct patient assessments in home, clinics, hospital or SNF settings.
• Coordinate services within patient plan eligibility using community resources.
• Collaborate with external and internal teams to improve patient and provider satisfaction.
• Participate in PI or regional strategic initiatives and IDT meetings.
• Maintain licensure, certifications and annual trainings.
