Oak Street Health is seeking a Medical Social Worker to support psychosocial needs of high-risk patients. You will assess, advocate, and coordinate care in collaboration with interdisciplinary teams to improve health outcomes and reduce hospitalizations.

Responsibilities include comprehensive assessments, care plan execution, and navigating insurance benefits while maintaining thorough documentation. Strong multilingual and EMR skills are highly valued in a fast-paced care environment.

Competenze

• Master of Social Work degree required from a CSWE-accredited program.
• Minimally licensed in the state where OSH Center is located.
• Fluency in Spanish, Polish, Russian, or other languages common to the served communities.
• Proficient computer skills (MS Office, EMR, and telehealth tools).
• US work authorization required.

• Provide comprehensive psychosocial assessments, advocacy, and care coordination for high-risk patients and caregivers.
• Collaborate with interdisciplinary teams to identify goals and drive care plan execution.
