You will advocate for families, link to community and financial resources, and collaborate with an interdisciplinary team to ensure optimal discharge planning and patient well-being.

The role requires a BS in Social Work and state certification, with preferred master's level credentials and 2+ years in healthcare.

Responsabilités

• Coordinate patient care and advocate for resources.
• Provide counseling and psychosocial support to patients and families.

• Assist with discharge planning and transition of care.
• Collaborate with care team to ensure quality outcomes.
• Maintain accurate documentation and reporting as required.
• Follow CMS and Joint Commission standards.
