Job summary

KenCrest is seeking an Outcome Specialist Coordinator for its Supported Independent Living program. You will partner with individuals, families, Supports Coordinators, and interdisciplinary teams to identify meaningful outcomes and monitor ISP goals.

Responsibilities include coordinating assessments, documenting progress, and compiling reports for audits while ensuring HIPAA compliance and data quality across diverse settings.

Qualifications

• Bachelor's degree in Human Services or related field is required.
• Two years of experience with individuals with intellectual disabilities and autism preferred.
• Strong communication, organization, documentation, and collaboration skills.
• Ability to use technology including Outlook, Word, Excel, Teams, and Evolv.

Responsibilities

• Partner with individuals, families, Supports Coordinators, and team members to coordinate services and promote person-centered support.
• Create and distribute Outcome Progress Notes aligned with ISP updates and proper documentation.
• Schedule and complete quarterly reviews and annual assessments with individuals and support teams.
• Monitor outcomes, progress, and action steps; compile data for audits and KPIs.
