Job summary

Peoples Home Health, LLC in Florida seeks an experienced Patient Care Coordinator LPN to oversee day-to-day hospice operations and coordinate care for patients and families. You will partner with the Clinical Director to uphold quality care standards and ensure smooth service delivery.

The role requires completing referrals, scheduling nurse and chaplain visits, maintaining EMR reporting, and supporting quality assurance initiatives.

Qualifications

• Graduate of an accredited LPN program and licensed in Florida preferred.
• Minimum one year nursing experience preferred.
• Two years home care or hospice experience preferable.
• Proficient in Word, Outlook, and Excel.
• Ability to prioritize time and make independent decisions.
• Able to operate nursing assessment tools and office equipment.
• Current CPR certification.
• Valid driver’s license and auto insurance.

Responsibilities

• Scheduling visits for Registered Nurses, Social Worker and Chaplains.
• Scheduling palliative care visits for the Nurse Practitioner.
• Ensuring staff complete visits in a timely manner.
• Serving as scribe for interdisciplinary team meetings.
• Gathering information to input a complete referral.
• Oversees coordination of patient care for Hospice.
• Assists with quality assurance and performance improvement.
• Runs EMR reports to ensure compliance and daily activity logging.
