Job summary

Endeavor Health Immediate Care is seeking an Advanced Practice Provider (APN/PA) for a part-time, outpatient role based in Evanston, IL and surrounding locations. The position offers 36 hours per week with shifts Mon–Fri between 8:00 AM and 8:00 PM and weekend/holiday rotation 8:00 AM–4:00 PM, focusing on autonomous, evidence-based care in a collaborative team.

The ideal candidate has an IL APN or PA license, DEA/CS license, a master’s degree in the specialty, and BLS/CPR certification; 2+ years

Qualifications

• In IL, APN or PA licensure required; DEA and Illinois Controlled Substance License required.
• Master’s level program in specialty area; FNP certification required for APRNs.
• BLS/CPR for Healthcare Provider certification required; in-person skills validation.

Responsibilities

• Under supervision of collaborating physicians, provides care to patients.
• Obtains history, performs exams, orders imaging and labs, interprets results.
• Treats patients, and documents progress notes and treatment plans.
• Collaborates with physicians and healthcare team to achieve optimal outcomes.
• Prescribes therapies and participates in standards of practice development.
