Job summary

NYAP is seeking a Family Services Coordinator to provide in-home support, advocacy, and referrals to youth and families, aiming to help children live safely in the community. The role involves collaboration with youth, foster parents, and community partners, and requires flexible hours and strong documentation.

Responsibilities include coordinating across mental health, medical, education, and social supports; daily community travel; maintaining timely records; and supporting family

Qualifications

  • Degree in Social Work or comparable Human Services field from an accredited institution.
  • Willingness to work flexible and non-traditional hours.
  • Capacity to work with youth, foster caregivers, and diverse backgrounds.
  • Proficient use of computers, smartphones, tablets, and basic software.

Responsibilities

  • Completes all required documentation and trainings promptly.
  • Coordinates and monitors services across mental health, medical, educational, psychological, vocational, and social supports.

  • Provides safety assessment, crisis response, and behavioral stabilization as needed.
  • Participates in Continuous Quality Improvement activities.

Intensive In- Home Services: Family Services CoordinatorCompensation: $46,000-48,000/year (based on experience)Position SummaryUnder the general supervision of the Clinical Supervisor or designee, this position works to make a meaningful difference in the lives of the children and families served.
