Job summary

NYAP seeks a Family Services Coordinator to advocate for youth and families in Preble County. Under supervision, you will provide in-home support, transportation, counseling, and referrals to help youth live safely in the community.

You will document regularly, coordinate services with foster parents and partners, and adjust plans to ensure progress toward goals. Flexibility for non-traditional hours is required.

Qualifications

  • Bachelor's degree in Social Work or comparable Human Services field from an accredited institution.
  • 2 years of work experience in direct service with youth and families preferred.
  • Flexibility to work non-traditional hours and adapt to family needs.
  • Ability to work with diverse cultures and communities.

Responsibilities

  • Provides in-home supportive services, transportation, parenting education, counseling, and community resource referrals.
  • Coordinates and monitors services for youth and family in the community across multiple domains.
  • Regularly reviews and updates youth-based plans to ensure progress toward goals.
  • Maintains timely documentation and completes trainings as required.
  • Travels daily to deliver community-based services in compliance with contracts and regulations.

IHBT Family Services CoordinatorCompensation: $48,000/yearTerritory: Preble CountyUnder the general supervision of the Clinical Supervisor or designee, this position works to make a meaningful difference in the lives of the children and families served.
