Wellmore Behavioral Health in Middlebury, CT seeks an In-Home Clinician to provide direct services to children, adolescents and families in home and community settings as part of the Intensive Home Based Therapy program.

You will conduct assessments, develop treatment plans, offer individual and family therapy, crisis intervention and care coordination, with a flexible schedule including evenings or weekends as needed.

Qualifications

  • Provide comprehensive assessments, treatment plans, and clinical services including individual, group and family therapy, case management, outreach and crisis intervention.
  • Implement treatment plans with support services and linkage to community resources.
  • Deliver home- and community-based services in homes, schools, agency sites and other community locations.
  • Maintain a flexible schedule including evenings, weekends, overnight and after-hours availability.
  • Maintain regular contact and follow-up with families including home visits and home-based therapy when required.
  • Keep accurate, confidential clinical and program documentation and meet productivity expectations.
  • Collaborate with multidisciplinary teams, families, schools and community partners in treatment planning and service coordination.
  • Engage families in treatment planning and promote active participation in service delivery.
  • Participate in program development, outreach, trainings, supervision and agency/community meetings.
  • Provide supervision to assigned staff and/or interns as applicable.
  • Perform other duties as assigned by supervision.

Job Details: Job Location: Middlebury, CT 06762, Position Type: Full Time, Salary Range: $63,860.00 - $76,000.00, POSITION TITLE: In-Home Clinician

PROGRAM:

Intensive Home Based Therapy program

GENERAL STATEMENT OF RESPONSIBILITIES:

The In-home Clinician is responsible for direct services with children, adolescents and their families including but not limited to evaluation/assessment; home & community based services; outreach; case and care management; individual and family therapies; crisis intervention; record keeping; collaboration with other professionals and team members and related community services (including consultation and/or community and/or school placements). He/she is assigned to carry a pager and responds to all calls for assistance either by telephone or in person (as clinically indicated). She/he provides the majority of treatment services in the home of clients and works a flexible schedule, as based on the needs of assigned cases.

SUPERVISION:

Supervised by: Program Coordinator

Supervises: Interns and/or those designated by supervisor.

EXAMPLES OF RESPONSIBILITIES:
  • Provide comprehensive assessments, treatment plans, and clinical services to children, adolescents, and families, including individual, group, and family therapy, case management, outreach, and crisis intervention.
  • Implement treatment plans and provide clinically required support services, including advocacy, care coordination, translation, transportation in accordance with agency policy, and linkage to community resources.
  • Provide primarily home- and community-based services in settings such as homes, schools, agency sites, emergency rooms, shelters, and other community locations, based on client and program needs.
  • Maintain a flexible schedule, including evenings, weekends, overnight, and after-hours emergency availability, as required by the program.
  • Maintain regular contact and follow-up with assigned families, including home visits and home-based therapy when required.
  • Maintain accurate, confidential, and timely clinical and program documentation and meet productivity expectations established by the program.
  • Collaborate with multidisciplinary teams, families, schools, and other community partners in treatment planning, case conferences, and service coordination.
  • Engage families in treatment planning and decision-making and promote their active participation in all aspects of service delivery.
  • Participate in program development, outreach, marketing, staff meetings, trainings, supervision, and agency/community committees as assigned.
  • Provide supervision to assigned staff and/or student interns, as applicable.
  • Perform other duties as assigned by the Supervisor.

Qualifications:

QUALIFICATIONS:
  • Must have the ability to maintain the utmost confidentiality of both agency and client matters. Must understand and be able to display the clinical knowledge and expertise necessary to perform crisis intervention. Must be able to display knowledge and expertise in clinical theory and practice as it relates to the populations served by Wellmore, Inc. Must be capable of working as a part of a team and be able to effectively communicate with both families and professionals (in writing and verbally). Must be willing to work a flexible schedule that meets the needs of assigned families. Must be able to display at least an understanding of the diverse cultural communities prominent in greater Waterbury. Working knowledge of the greaterWaterbury area is a must! Bi-lingual (Spanish-English) may be required.
EDUCATION & EXPERIENCE:

A Master’s Degree in marriage and family therapy, social work or related human services field with at least a one year clinical internship working with children and/or adolescents or families is required.A Connecticut license as a Marriage and Family Therapist, Professional Counselor, Licensed Clinical Social Worker or Doctorate Psychologist required. MSW candidates must possess a LMSW at a minimum. Two years post graduate clinical experience working with children, adolescents or families is preferred. Experience in family therapies as well as family systems theory and practice is preferred.

LICENSURE/CERTIFICATION/REGISTRATION:

Valid driver’s license, reliable transportation and valid auto insurance; CPR and first-aid.
