Job summary

National Youth Advocacy Program (NYAP) in Fairfield County, Ohio is seeking a full-time Community Based Therapist to support children and families through Home Based Therapy services. You will provide counseling, advocacy, and coordination to help youth live safely in the community.

The role requires a Master’s in Social Work or related field, a valid professional license, and willingness to travel daily in the service area.

Qualifications

Master’s Degree in Social Work or comparable Human Services field from an accredited institution.
2 years of work experience working in direct service with youth and families strongly preferred.
A valid advanced professional license as required and acceptable by state or contract regulations.

Responsibilities

Provides individual, group, and family mental health services, case management, transportation, and other professional services, meeting or exceeding established outcomes thresholds.
Travels daily to provide community based services to youth and families in compliance with organizational, contract, and regulatory requirements.
Completes all required documentation in a timely manner.
Consistently achieves established productivity thresholds.
Participates in on-call rotation to provide crisis support and crisis intervention for IHBT families.
Coordinate and monitor services for the youth and family in the community, including mental health medical, educational, psychological, vocational, and social services.
