Job summary

NYAP is seeking a Community Based Therapist to join the IHBT team serving Franklin County, Ohio. You will provide individual, family, and group therapy, along with case management and crisis support, to youth and families in the community.

Requires a Master’s in Social Work or related field, a valid advanced license, and a driving record suitable for daily community travel. The role offers comprehensive benefits and opportunities for professional growth.

Qualifications

• Master’s Degree in Social Work or comparable Human Services field from an accredited institution.
• 2 years of direct service with youth and families strongly preferred.
• Valid advanced professional license and a valid driver’s license.

Responsibilities

• Provides individual, group, and family mental health services and case management.
• Travels daily to deliver community-based services to youth and families.
• Completes documentation in a timely manner and meets productivity thresholds.
• Participates in on-call crisis support and crisis intervention.
• Coordinates services across mental health, educational, and social systems.
• Reviews and updates youth plans to ensure progress toward goals.
• Builds strong relationships with foster parents and families.
• Assists in family reunification and stabilization efforts as needed.
• Performs safety assessments and crisis stabilization when required.
• Participates in Continuous Quality Improvement and weekly supervision.
