Job summary

Parker Personal Care Homes in Lakewood, CO is seeking a Residential Program Coordinator to enhance quality of life for customers with IDD who receive services in family homes or from paid family members. You will act as liaison among individuals, caregivers, families and the team to support goals and choices.

The role requires at least one year of IDD experience, a high school diploma, strong communication and computer skills, and a valid driver’s license.

Qualifications

• High school diploma or equivalent required.
• One or more years of work experience in IDD field required.
• Experience in Colorado Developmental Disabilities System preferred.
• Must have valid driver's license.

Responsibilities

• Coordinate Host Home services and oversee care plans.
• Ensure compliance with PPCH policies and state/federal regulations.
• Maintain Host Home documentation and health-related records.
• Attend meetings and coordinate health-related services documentation.
• Respond to external/internal auditors' requests.
