Thresholds seeks an OBRA Assessor to perform psychosocial and psychiatric assessments for adults with intellectual and developmental disabilities referred for Pre-Admission Screenings and Annual Resident Reviews in Kent County nursing facilities. The role is hybrid and PRN, with collaboration across OBRA staff.

The ideal candidate holds Michigan licenses (LP/LLP/LLMSW/LMSW, LPC/LLPC, RN with psychiatric focus) and can work flexible hours, delivering timely assessments and coordinating care with

• Active Michigan license or credential as listed in qualifications.
• Ability to complete assessments within short timelines.
• Experience working with adults with intellectual or developmental disabilities preferred.

Responsabilidades

• Complete comprehensive psychosocial and psychiatric assessments for Pre-Admission Screenings and Annual Resident Reviews for adults in nursing facilities.
• Coordinate and communicate assessments with OBRA staff and report recommendations.
• Provide consultation and training to nursing home staff and other agencies as part of the OBRA team.
• Comply with policies and documentation requirements of the facilities and agencies.
