Job summary

Thresholds Inc. is seeking an OBRA Registered Nurse to perform comprehensive nursing assessments for adults with intellectual and developmental disabilities referred for Pre-Admission Screenings and Annual Resident Reviews in Kent County nursing facilities.

The RN will collaborate with OBRA staff, provide guidance to nursing home teams, ensure compliance with applicable policies, and document findings for reporting and planning.

Qualifications

• MI RN license is required.
• Experience with adults with IDD preferred.
• Valid Michigan drivers license and access to a personal vehicle.
• Willingness to work flexible hours.
• Ability to work well in a team.
• Knowledge of adult psychiatric illness preferred.

Responsibilities

• Complete comprehensive nursing assessments for Annual Resident Reviews of adults in nursing homes identified as intellectually or developmentally disabled.
• Perform Pre-Admission Screenings for adults identified as intellectually or developmentally disabled.
• Collaborate with OBRA staff to coordinate assessments and report recommendations.
• Provide consultation and training to nursing home staff as part of the OBRA team.
• Ensure compliance with Thresholds and applicable state/federal policies and procedures.
• Represent the agency in meetings or trainings as needed.
• Complete documentation according to nursing home and agency requirements.
• Perform other duties as assigned by supervisor.

POSITION SUMMARY

The nurse will complete comprehensive nursing assessments, as needed for adults with Intellectual and Developmental Disabilities, who are referred for Pre-Admission Screenings, as required for placement into nursing facilities. The nurse will also complete nursing assessments, as needed, for Annual Resident Reviews for adults served by Thresholds who are currently residing in nursing facilities in Kent County.

QUALIFICATIONS

• RN licensed to practice in the State of Michigan
• Experience working with adults who have an intellectual or developmental disability preferred.
• Valid and legal Michigan driver's license and personal vehicle required.
• Ability and willingness to work flexible hours required.
• Ability to work effectively within a team context is essential
• Knowledge of adult psychiatric illness including psychotherapeutic medications preferred.

REQUIREMENTS

• Independently drives personal car.
• Communicates both orally and in writing with persons served, other agencies, co-workers, etc.
• Engages in meetings and counseling sessions with a high level of concentration over extended periods of time.
• Understands and follows instructions.
• Normal hearing required for conversation, telephone use, and emergency information systems.
• Visual acuity, depth perception, and field of vision within normal range.
• Must be independently mobile on all surfaces and situations.
• Must be able to move safely about work area.
• Must be able to navigate online appointments/meetings via Zoom, Go-to meetings, or Teams.

KEY RESPONSIBILITIES

• Complete comprehensive nursing assessments, as needed, for Annual Resident Reviews of adults residing in nursing homes who are identified as intellectually or developmentally disabled
• Complete comprehensive nursing assessments, as needed, for Pre-Admission Screenings for adults who are identified as intellectually or developmentally disabled.
• Works in conjunction with other agency OBRA staff to effectively coordinate and communicate assessments and report recommendations
• May provide consultation and training to nursing home staff, as part of the OBRA team.
• Comply with policies, procedures and standards established by Thresholds, network180, the Lakeshore Regional Entity, MDHHS and relevant Federal, State, and City Departments.
• Represent the Agency in various meetings, speaking engagements, or trainings as needed.
• Comply with all documentation requirements on behalf of the person-served in accordance with the applicable policies and procedures of the Nursing Home, Thresholds, or the local or State Mental Health Agencies.
• Perform other related duties and responsibilities as assigned by the immediate supervisor or other agency administrator.
