Job summary

IAHD in Bronx, NY is seeking an experienced professional to coordinate individualized plans and lead interdisciplinary teams for clients with developmental disabilities. The role emphasizes person-centered planning and progress tracking to support growth.

Responsibilities include monitoring progress, ensuring OPWDD compliance, helping clients gain independence in daily living and social skills, and contributing to staff training programs.

Qualifications

• Bachelor’s degree in a Human Services field required.
• 1+ years of relevant experience preferred or 1 year of DSP experience.
• Proficiency in person-centered planning and progress tracking.
• Understanding of OPWDD regulations and documentation.

Responsibilities

• Coordinate Personalized Plans: develop and implement individualized plans for each client, including IPOP and other plans.
• Lead Interdisciplinary Teams: facilitate team meetings to ensure holistic, quality active treatment.
• Monitor Progress: track and evaluate client goals and adjust plans as needed.
• Ensure Compliance: maintain up-to-date electronic records compliant with OPWDD regulations.
• Support Independence: assist clients in life skills, social interactions, and personal goals.
• Provide Input for Training Programs: help develop and monitor staff training programs.
