Job summary

DON GUANELLA VILLAGE in Norwood, PA seeks a full-time position focused on comprehensive developmental services. The role leads the development and monitoring of individual program plans and coordinates with an interdisciplinary team to ensure active treatment and appropriate supports.

Responsibilities include intake, assessment coordination, plan progress tracking, and interdepartmental communication, with attention to licensing and rights advocacy.

Qualifications

• Bachelor’s degree in a human services field (including sociology, special education, rehabilitation counseling, and psychology).
• Two years’ experience working with people with Intellectual Disabilities.
• Must communicate effectively in English.

Responsibilities

• Participates in intake and admission of new individuals.
• Acts as Plan Lead in development, implementation and monitoring of each individual’s program plan.
• Supervises, monitors and evaluates services, equipment, and programs to meet needs and ensure Active Treatment.
• Coordinates initial and subsequent assessments for each individual on caseload.
• Monitors progress of goals and documents quarterly progress reports.
• Communicates progress and needs to the Plan Team and Supports Coordinators.
• Collaborates with residential, nursing, day program, dietary, training, county supports coordinators and families.
