Job summary

JCFS Chicago seeks a Medical Services Coordinator to facilitate medical and related supports for individuals with Intellectual and Developmental Disabilities, focusing on personal outcomes and independence. You will schedule appointments, document outcomes, assist with medications under RN guidance, and coordinate with providers, families, and staff to ensure timely care.

Requirements include HS diploma, TABE/STOFHLA, IL driver license, CPR/First Aid, DSP training; CNA preferred; 2+ years in

Qualifications

• High School Diploma or GED required.
• TABE or STOFHLA literacy assessments.
• CPR and First Aid completion.
• DSP curriculum completion per DHS.
• Valid Illinois Driver's license with clean record.
• CNA certification preferred; 2+ years in human services.

Responsibilities

• Schedule, facilitate and follow up on medical appointments per DHS requirements.
• Collaborate with RN trainer and provide written report on outcomes.
• Transcribe new medication orders and assist with medication delivery.
• Manage medical closets, First Aid, PRN and MAR documentation.
• Facilitate transportation and supports during emergent medical needs.
• Provide DSP trainings and on-call support as part of rotation.
• Attend team meetings and maintain required trainings.
