Texoma Community Center in Sherman, TX seeks an AMH IDD Dual Case Manager to provide rehabilitation case management and service coordination for individuals with persistent mental illness and co-occurring intellectual/developmental disabilities. Qualifications

• Bachelor’s degree in social work, sociology, psychology or counseling with two years human services experience.
• Preferred one year experience with persons with intellectual disability or related conditions.
• Current driver’s license and vehicle for work; QMHP/QIDP eligibility within 90 days.

Responsibilities

• Provide rehabilitation case management or service coordination as outlined by state guidelines.
• Advocate, deliver crisis services and psychosocial rehabilitation following trauma-informed models.
