Job summary

Bluebonnet Trails Community Services in Round Rock, TX is seeking a Residential Case Manager for the DDRF to deliver person-centered case management for adults with severe mental health and co-occurring substance use disorders housed at the facility.

The position is Monday through Friday, non-exempt, with no night shifts or on-call requirements, reporting to the TCOOMMI Program Director and collaborating with LCDCs. Strong documentation and adherence to state guidelines are essential.

Qualifications

• Bachelor's degree in human services.
• Two years of comprehensive knowledge of mental health and treatment.
• Valid Texas Driver's License and driving status throughout employment.
• NCIC/TCIC background checks, substance use screening, and fingerprinting are performed prior to employment.
• An official copy of the college transcript is required.
• If a degree is not in Human Services, a minimum of 24 college credits in a Human Services field are required.

Responsibilities

• Provide case management, skills training and psychosocial rehabilitative services.
• Collaborate with LCDCs to develop individualized treatment plans.
• Stabilize mental health and substance use symptoms and reduce recidivism risk.
• Maintain detailed electronic documentation according to guidelines.
• Transport to BCBS clinic and other transportation needs as identified.
