Job summary

Charles Lea Center is seeking a compassionate Community Facilitator to support adults with IDD in building connections within their communities. The role focuses on promoting independence, social engagement, inclusion, and self-directed living.

You will help individuals access resources, participate in activities, and build relationships in residential and community settings, while documenting progress and coordinating programs with partners.

Qualifications

• Associate or bachelor’s degree preferred.
• Minimum of 2 years IDD experience.
• Background in Human services is a plus, primarily in residential & community settings.
• Good computer skills to include Microsoft Office.

Responsibilities

• Build meaningful relationships with community organizations and local resources.
• Foster an inclusive and welcoming environment for all individuals.
• Encourage participation in community activities and help develop social connections.
• Help individuals gain confidence in interacting with others and navigating community settings.
• Identify volunteer, recreational, educational, and employment opportunities aligned with goals.
• Plan, coordinate, and facilitate community events, workshops, and networking opportunities.
• Develop creative programs that promote community integration and social engagement.
• Collaborate with program leadership and community partners to enhance opportunities.
• Provide education on community resources, safety, wellness, and amenities.
• Assist individuals in building life skills and independence.
• Support transportation planning, including public transit navigation and paratransit resources.
• Educate individuals, families, and caregivers on community safety and well-being.
• Implement and follow residential, behavioral, and support plans.
• Track and document individual progress toward goals and outcomes.
