Job summary

Dentonmhmr seeks a Collaborative Care Case Manager (CCCM) to oversee intensive case management for individuals with IDD and co-occurring mental health or substance use conditions in the Outpatient Biopsychosocial Interventions program. You will conduct biopsychosocial assessments and develop person-centered recovery plans, coordinating with providers to ensure comprehensive treatment.

Qualifications

• Knowledge of intellectual and developmental disabilities and related services.
• Ability to communicate effectively with clients, families, and providers.
• Commitment to trauma-informed care principles.

Responsibilities

• Perform assessments necessary for enrollment into the program in a timely manner.
• Establish and maintain the person centered recovery plan for individuals served.
• Perform intensive case management for individuals assigned to caseloads; see participants at least twice a month.
• Complete skills training activities aligned with treatment goals (at least monthly).
• Collaborate with LIDDA/LMHA staff, Transition Support Team, and community resources.
• Attend weekly team meetings.
• Complete EHR documentation within two business days of meetings.
• Adhere to Texas Administrative Code guidelines and HHSC requirements.
• Participates in the Center's Zero Suicide Initiative and crisis interventions as needed.
