The role requires a high school diploma with two years’ IDD experience or a bachelor’s degree, a valid Texas driver’s license, and strong writing, time management, and verbal skills.

Qualifications

  • High school diploma or bachelor’s degree with relevant case management experience per level.
  • Proficient in Microsoft Word and Outlook.
  • Ability to manage own caseload independently and meet deadlines.
  • Must have a valid Texas driver’s license and be insurable.
  • Spanish language ability preferred.

Responsibilities

  • Maintain ethical and professional communications using Trauma-Informed Care language.
  • Assist individuals with IDD in their homes and communities to promote independence.
  • Collaborate with team to ensure safety, happiness, and well-being of individuals served.
  • Meet with assigned caseload monthly to monitor services and coordinate supports.
  • Document and update PDPs, service plans, and notes by deadlines.

Essential duties and responsibilities:

  • Staff will maintain ethical and professional communications at all times using respectful Trauma-Informed Care language
  • Making lives better for individuals with Intellectual Developmental Disabilities both in their homes and in their community
  • Work within a team to ensure the overall safety, happiness, and well-being for the individuals we serve
  • Meet with individuals on assigned caseload at least monthly to monitor services requested, that may include day programs for socialization/education/community integration, training in the home to increase independence, transportation, respite, nursing, dental, and group home/host home
  • Participate in team meetings to develop the Person Directed Plan (PDP) annually
  • Build community resources and connect individuals we serve so they can live as independently as possible
  • IDD serves individuals in the top 21 counties of the Texas Panhandle, so some travel may be required during the workday to meet with individuals on your assigned caseload
  • In compliance with state regulations, documentation must be entered/completed according to program deadlines and company policies
  • Adheres to TPC Business Code of Conduct and reports concern if ethical and/or business standards defined in the code are compromised

Required qualifications to be successful in the job:

  • For Case Manager I, high school diploma or equivalent with two (2) years of experience working with individuals with IDD, OR a bachelor's degree in any field with no case management experience
  • For Case Manager II, bachelor's degree in any field with a minimum one (1) year of professional experience in case management
  • Graduate work may be substituted for up to one (1) year of experience
  • Computer skills including Microsoft Word and Outlook
  • Good writing, time management, and verbal skills
  • Ability to manage your own caseload independently
  • The ability to organize documents for assigned caseload and meet time deadlines

  • Must have a valid Texas driver’s license and be insurable under agency vehicle policy
  • Must be 18 years of age or older

Preferred qualifications to be successful in the job:

  • Bachelor's degree from an accredited college or university
  • Previous work experience with individuals with Intellectual Disabilities
  • Previous experience using SmartCare EHR
  • Ability to speak Spanish
