Qualifications

• Active NP or PA certification with an unrestricted license in the practicing state.

• Ability to conduct comprehensive in-home assessments across Idaho.

Responsibilities

• Perform in-home health assessments for Medicare Advantage members.
• Document diagnoses and coordinate with PCPs to close care gaps.
• Educate patients on disease management and medication use; address social needs.
