Job summary

Optum Home & Community Care is seeking a HouseCalls Advanced Practice Clinician (NP/PA) to travel statewide in Idaho for in-home health assessments and coordinated care for Medicare Advantage members. The role emphasizes education, gap closure, and collaboration with primary care providers to improve health outcomes.

Live anywhere in Idaho with a flexible 8am–8pm schedule over a 7-day week. Travel expenses are covered and compensation includes a base salary with incentives and a sign-on option.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active and unrestricted licensure in the practicing state without disciplinary actions.
• Access to reliable transportation for home visits and travel across Idaho.

Responsibilities

• Conduct in-home health assessments for Medicare Advantage and other plan members.
• Educate patients on disease processes, medications, and care plans.
• Review past medical history, vital signs, and perform physical exams.
• Identify care gaps and coordinate with PCPs to close them.
• Document findings and participate in plan-based care management.
• Recognize urgent situations and intervene as needed.
• Assist with referrals addressing social determinants of health.
