Job summary

Optum Home & Community Care seeks a Nurse Practitioner for HouseCalls statewide travel across Idaho. This field-based role performs annual in-home assessments for Medicare Advantage members, with no on-call, weekends, or holidays.

Competitive compensation includes base pay plus incentives and a $40,000 student loan repayment or $25,000 sign-on bonus. Travel about 85% across the state is required; expenses are covered.

Qualifications

• Active NP certification (ANCC/AANP) in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted NP license in Idaho or ability to obtain by start date.

Responsibilities

• Perform in-home health assessments and vital signs.
• Document diagnoses for care management and treatment planning.
• Educate patients on disease processes, medications and adherence.
• Coordinate with PCPs to close care gaps and ensure optimal outcomes.
• Identify urgent situations and intervene appropriately.
