Job summary

PATH (People Acting to Help), Inc. is seeking dedicated staff to promote the mission of helping individuals achieve a more independent and fulfilling life.

The role involves training and supporting program participants as part of a collaborative team in a comprehensive behavioral health setting. Qualifications include a HS diploma or equivalent, a valid driver's license with access to a vehicle, and at least one year of experience with individuals with disabilities.

Qualifications

  • HS Diploma or equivalent is required.
  • Valid driver's license and a reliable vehicle are required.
  • At least one year of experience with individuals with disabilities.

Responsibilities

  • Coordinate training and activity planning for individuals in the program.
  • Assist with annual DPW Licensing standards preparations.
  • Meet productivity standards of the ADP Services.
  • Assist in developing assessments for assigned individuals.
  • Attend team, ISP, QEST, SIS, Vision for Equality meetings.
  • Explore and facilitate community integration opportunities for individuals.
  • Meet regularly for supervision with your supervisor.
  • Participate in related investigations with Human Resources.
  • Assist individuals to and from the van each day.
  • Provide assistance and supervision of individuals with personal care during activities.
  • Drive the van for programmatic reasons (as needed).
  • Attend staff meetings.

JOB SUMMARY:

To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. Responsible for the training and support of individuals in the program.

Specific Duties
  • Assist in the coordination of training and activity planning for individuals in the program.
  • Assist with the preparation for annual DPW Licensing standards and Office of Long-Term Living Licensing.
  • Meet productivity standards of the ADP Services.
  • Assist in the development of assessments for assigned individuals.
  • Attend, as needed, individual team meetings, ISP meetings, QEST reviews, SIS meetings, Vision for Equality monitoring and Supports Coordination monitoring.
  • Explore and facilitate community integration opportunities for individuals.
  • Meet regularly for supervision with your supervisor.
  • In conjunction with Human Resources, participate in related investigations.
  • Assist individuals to and from the van each day.
  • Provide assistance and supervision of individuals with personal care during group and individualized activities.
  • Drive the van for programmatic reasons (during program hours) on an as-needed basis.
  • Complete assigned responsibilities related to the upkeep of the facility.
  • Attend staff meetings
Qualifications

  • Valid driver's license and vehicle required.
  • At least one year of experience with individuals with disabilities.
