Job summary

Trinity Health’s Pittsburgh Mercy operates an Intellectual and Developmental Disabilities program where an ID Floater provides compassionate, individualized care in residential settings. You will partner with families and interdisciplinary teams to support residents’ well-being and choice-driven life goals.

The role emphasizes daily living assistance, care planning, and advocacy, with an evening shift from 2:30 pm to 10 pm and a focus on community integration and safety.

Qualifications

• Bachelor’s degree OR High School Diploma/Equivalence and 1 year of paid social services work experience
• Valid Driver License and ability to drive an organization vehicle
• Access to transportation travel locally
• Act 33/34/73 Clearances
• Pre-Employment Drug Screen, Physical/TB
• Must successfully pass Department of Public Welfare Medication Training within 90 days of employment

Responsibilities

• Individualized Care Planning: Develop and implement individualized care plans based on assessments.
• Daily Living Assistance: Provide hands-on assistance with daily activities, meals, medication management, and recreation.
• Emotional and Behavioral Support: Offer empathetic support and de-escalation to residents.
• Community Integration: Organize social and recreational activities to promote belonging.
• Advocacy and Collaboration: Liaise with external agencies, families, and healthcare professionals for holistic care.
