Qualifications

• Nursing diploma or associate's degree.
• Current RN license.
• 2–5 years of nursing experience.
• Bachelor's degree in Nursing preferred.
• 5–10 years of nursing experience preferred.

Responsabilités

• Facilitates identification and resolution of gaps in care from hospitalization, at discharge, and into the ambulatory care setting.
• Leads the development and implementation of complex individual plans of care with defined goals in collaboration with other members of the interprofessional team and patient/family.
• Participates in journal clubs, supports evidence-based practice, and disseminates new knowledge.
• Evaluates patient and family experience and adjusts care to improve satisfaction and quality of care.
• Works as a safety-sensitive employee in a high-stakes clinical environment.
