Qualifications

• RN with Nursing Diploma or Associate's degree
• Bachelor's degree in Nursing preferred
• 2–5 years nursing experience
• RN licensure

Responsibilities

• Facilitates identification and resolution of gaps in care from hospitalization, at discharge, and into the ambulatory care setting.
• Leads development and implementation of complex care plans with defined goals in collaboration with an interprofessional team and patient/family.
• Promotes evidence-based practice and supports standards of care and professional development.
• Evaluates patient and family experience to improve satisfaction and quality of care.
• Maintains safety-sensitive requirements and adheres to safety guidelines.
