Job summary

Responsibilities

• Coordinate holistic care plans by partnering across multidisciplinary teams to deliver seamless, well-managed care journeys for every patient.
• Monitor and communicate patient progress in real time to ensure prompt medical interventions and keep families informed.
• Execute physician and nursing orders with precision to maintain safety and treatment accuracy.
• Assess and drive discharge planning with the healthcare team for a smooth transition home.
