• Performs plan of care, medications, specimen collection, treatments and other medical care.
• Documents patient care and communicates changes to the care team.
• Monitors patient condition and collaborates with the team and family.
• Assists with activities of daily living and safety needs.
• Coordinate transfers, discharges and referrals; provide education.
• Advocates for patient wellbeing and supports family education.
