Job summary

Responsibilities

• Performs Nursing Assessment on admission, at each shift and with change of condition.
• Develops Plans of Care with individualized interventions and documents outcomes.
• Administers medication and IV therapy per policy; monitors patient condition.
• Transcribes and signs off orders; maintains proper chart documentation.
• Coordinates discharge planning with Case Manager and physicians.
