Responsibilities

• Follows policies and procedures and uses Nursing Process.
• Conducts thorough Nursing Assessments at admission and with change of condition.
• Develops and updates Plans of Care with expected outcomes.
• Administers medications and IV therapy per policy; ensures MD orders are complete.
• Documents in all required forms and charts; maintains accurate records.
• Provides patient/family education and discharge planning coordination.
• Collaborates with Case Management and physicians for safe transfers and discharge.
