Qualifications

Responsibilities

• Assesses and analyzes the health status of patients in assigned nursing unit.
• Performs and documents assessments on all admitted patients.
• Plans patient care with patient, family, and health care team.
• Directs, implements and coordinates patient care activities on the unit.
• Provides patient education based on assessed needs and evaluates understanding.
• Participates in Performance Improvement activities and Core Measures.
