Responsibilities

• Performs a comprehensive assessment on all assigned patients with documentation.
• Communicates changes in patient condition promptly.
• Documents outcomes and updates care plans.
• Collaborates with patient, family and health care team on discharge planning and referrals.
• Maintains accurate bedside hand-off and rounds with care team.
