Qualifications

Responsibilities

• Transcribes and administers medications and IV therapy per policy.
• Completes Nursing Assessment at admission and with changes of condition.
• Develops Plans of Care with individualized interventions and outcomes.
• Communicates effectively with patient and family; provides education.
• Ensures complete and accurate documentation on all forms.
• Coordinates discharge planning with Case Manager and physicians.
