Job summary

Jefferson Health Northeast in Langhorne, PA is seeking a Registered Nurse to deliver patient- and family-centered care using the nursing process. You will develop care plans with patients and designated care partners, addressing physical, psychological, emotional, and social needs while respecting cultural beliefs and values.

Minimum qualifications include an active PA RN license and a Specialized Diploma (BSN strongly preferred). BLS certification and acute-care experience are highly valued.

Qualifications

• Specialized Diploma; graduate of an accredited nursing program.
• Knowledge of patient- and family-centered care and cultural awareness.
• Effective communication and collaboration with patients and families.
• Knowledge of patient assessment, diagnosis, and care planning.
• Ability to educate patients and care partners.
• Understanding of discharge planning and post-discharge needs.
• Knowledge of health information documentation.

Responsibilities

• Interacts with coworkers, patients and families to deliver patient-centered care.
• Applies evidence-based practice and clinical expertise in care delivery.
• Educates patients and designated care partners on condition, treatment, and follow-up.
• Uses information technology to communicate, manage knowledge, and mitigate errors.
• Participates in safety and quality improvement committees.
• Fosters open communication and teamwork in inter-professional settings.
• Supports discharge planning and follow-up care arrangements.
