Responsibilities

• Interacts with patients, families and staff to deliver optimal healthcare

• Participates in safety and quality improvement initiatives
• Follows nursing practices and protocols to minimize harm and patient risk
• Performs discharge planning and patient education as part of care transition
• Maintains documentation in health information systems
