ICM Case Manager

Job summary

TURN is seeking an ICM Case Manager in Santee, CA to recommend access to medical, educational, social, vocational, housing, and more to support program goals. You will initiate referrals to next level care and coordinate with physical health providers for whole-person care while advocating for clients.

You will provide crisis intervention, discharge planning, individual and group counseling, and ensure documentation compliance.

Qualifications

• Master's degree; or bachelor's degree with four years of experience in social services.
• For SUD CM a certification from recognized board is preferred and registration required and a negative tuberculosis test (TB) is conducted under licensed medical supervision within 45 working days of hire or within 5 working days of employment and valid CPR and first aid certification upon hire or within 90-days of employment.
• For Housing two years of experience and an AA degree in social service or related field or High School Diploma and 5 years’ experience.
• Valid CA driver’s license and current auto insurance per contract requirements required.
• Experience providing mental health and/or substance use disorder services preferred.
• Knowledge of Microsoft Office and EHR software preferred.

Responsibilities

• Provide case management with crisis intervention, discharge plan, group & individual work, mental health/substance use disorder counseling, screening, and assist clients in meeting basic needs and symptom management.
• Complete accurate and timely documentation, meet minimum billable requirements for direct client services if contractually required.
• Facilitate placement including pre-placement visits, and transportation as needed.
• Shift coverage including part of the on-call team.
• Provide field-based services as required by program and client need.
• Maintain confidentiality of client’s care.
• Participate in scheduled program meetings.
• Other relevant duties as assigned.
• Collaborate closely with ACT team members and Program Manager to support smooth transitions between ACT and ICM levels of care.
