Job summary

Mental Health Systems, Inc. is seeking a Clinical Case Manager in Oceanside, California for the North Coastal ACT program.

The role is Regular Full-time (over 30 hours) to coordinate access to medical, educational, social, vocational, housing resources and advocate for clients. Candidates must have a Master’s degree in a behavioral science with valid BBS licensure (MFT, LCSW, LPCC) and a CA driver’s license; experience in mental health or substance use services is preferred.

Qualifications

• Master’s degree in behavioral science with valid BBS registration or license (MFT, LCSW, LPCC) required.
• Valid CA driver’s license and current auto insurance per contract requirements.
• Experience providing mental health and/or substance use disorder services preferred.
• Knowledge of Microsoft Office and EHR software preferred.

Responsibilities

• Provide case management with crisis intervention, discharge planning, group & individual therapy, counseling, screening and assist clients in meeting basic needs.
• Complete accurate and timely documentation, meet minimum billable requirements for direct client services if contractually required.
• Facilitate placement including pre-placement visits and transportation as needed.
• Shift coverage including part of the on-call team.
• Provide field-based services as required by program and client need.
• Maintain confidentiality of client’s care.
• Participate in scheduled program meetings.
• Other relevant duties as assigned.
• Support clients in court and collaborate with CARE team providers.

JOB SUMMARY

Recommend and assist access to medical, educational, social, vocational, housing, and more to support program goals. Initiate referrals to next level care, coordinate with physical health providers for whole-person care, and advocate in the best interest of clients.
